FAQ

Below are some of the questions that we are asked most frequently, from both our new and
established patients. If you do not see your question on the list, please do not hesitate to call
our office and speak to one of our staff members who will be happy to answer your question.


• Question: What do I need to bring with me to my first appointment?


Answer: If you will kindly bring:

•  Your driver’s license or other acceptable form of photo identification (passport, school ID),

    your insurance card.

•  Your referral or prescription from your primary care doctor (if indicated)

•  Any recent lab work or radiology studies.

•  A List of ALL of your current medications, including the dosage amounts.

 


• Question: Do I need a referral or authorization for my visit?


Answer: If Your Particular Insurance Plan requires that you have a referral to see a specialist,
you must go through your PRIMARY CARE PHYSICIAN and ask them for that referral. ONLY A
PRIMARY CARE PHYSICIAN can give you a referral. Other specialists may give you a prescription
that says “Urology Consultation” or something similar, HOWEVER, THIS IS NOT A REFERRAL.
When in doubt, call the phone number on the back of your insurance card and ask what the
protocol is for your plan.


• Question: Can I get a same day appointment?


Answer: Yes, if you are having an urgent issue. However, we will be fitting you in between patients with scheduled
times, so there may be a wait.


• Question: What do I have to pay on my first visit?


Answer: You will be responsible for any co-payments, deductibles, coinsurances or past due
balances prior to seeing the physician. We accept cash, check, Visa, Mastercard and American
Express.


• Question: Why do I have to pay a co pay each time I come in, even if it is for a follow-up?


Answer: Every time you see the doctor, even if it is two days in a row, he has to document
the visit and bill your insurance. Collection of co-payments is required by law as part of your

contract with your insurance company.
